*   ** ENROLLMENT CLOSED ** Please enroll me in the Voluntary Fixed-Price Plan. I understand that I must remain enrolled for 12 months, that my gas cost will be $8.889 per Dekatherm for the 12-month period from November 2007 through October 2008, and that my gas usage will be adjusted under ONG's Temperature Adjustment Clause.
 
*   Please enroll me in the Average Payment Plan . I have met the requirements of currently having no past due balance and I have resided at the address for 12 months or more. I understand my monthly bill is based on a 12-month rolling average of my natural gas bills and that ONG will make necessary adjustment to my budget payment amount each month based on the balance of my account.
 
*   ** ENROLLMENT CLOSED ** Please enroll me in the Budget Plus Plan. I understand selecting this option enrolls me in both the Voluntary Fixed-Price Plan and the Budget Payment Plan.
